Members will be assigned a registered nurse (RN) personal care manager to provide them with the individual support and assistance you need. He or she will work with primary care physicians with additional support from other care providers as necessary.<\/p>\n
Care Coordination team may also include social workers, community resource aides (navigators), and clinical pharmacists. Along every step toward reaching member health care goals, they will become familiar faces.<\/p>\n
Care teams consider members a whole person and not just as a \u201cmedical condition.\u201d They strive to help improve health and well-being.<\/p>\n
Members can meet with their Care Coordination team in person or over
the phone. In addition, they may even attend doctor appointments with a member to ensure all of their questions
are answered.<\/p>\n
Dignity Health\u2019s clinically integrated networks ensure members have flexibility in providers and a choice of whom to see for care, so they receive the right care at the right time in the right place.<\/p>\n
Participation in the program will not affect a member’s health benefits. When one decides to participate, they will receive a call to set up an appointment and discuss personal health care needs.<\/p>\n
Participation is voluntary and confidential. Only the member, physician and care team know you are enrolled in the program. We will not use or disclose member health information for anything but Care Coordination purposes. Any additional use will require member written authorization.<\/p>\n
